i had that happen on an f150 monster truck POS i had. but mine was from overtightening. Didnt look at what the impact was set at, pulled the lugnuts through the rim. even though it was me, still the trucks fault. that thing was garbage.
Had this happen in my early days of driving on an S-10, not a good feeling at all and I also learned the lesson of rechecking lugs.
