The A/c fan only works on high #5 - not on 1, 2, 3, or 4. Fan is smoothe - not noisy. Is there a fuse somewhere that I can check?

blower motor resistor, very common.

^^^ This. The high speed circuit for the blower motor is on its own fuse and and doesn't use the resistors, which is why it works in your case. There are several resistor styles available depending on which HVAC control you have (manual or automatic). Apparently you have manual controls.
