Misfire problem

What do you think caused the carbon buildup on one valve only? Leaky injector or worn plug?

I think it was just age. Machine shop said it was common. That truck was owned by a retired friend. He drove it like a Grandma. Probably never went over 2500 RPM even when he towed with it. I suspect that contributed.
