Exhaust leak. Lifter. Timing chain. Worn pushrod, rocker trunion. Serpentine pulley bearing. Flex plate crack.

A stethoscope comes in handy to narrow it down. (Harbor freight has them cheap)

Yes sir. Anything after that is going to be considered major work. It really could be anything so stethoscope is a good idea and narrow down where it's coming from.
