Carbon pellets leaking from evap canister? Those little suckers migrate like termites throughout your entire fuel system if the leaky canister isn’t caught in time.

Usually it doesn’t leave a DTC, just a poorly running engine, stalls and gas fill nozzle won’t stay on and constantly clicks off.
The canister is a new GM canister and thank goodness the old one didn’t leak pellets

How do you check this issue (leaking/migrating pellets)?

I'm wondering if I'm suffering from this. I have no fuel/evap codes, but there are times when I have teh three symptoms you just listed.

Pull evap canister back by fuel tank and see if they fall out. Have replacement available to install regardless.

I've got the new fluid to do my rear-diff, so I'll add that to the list of stuff to poke at while I'm under the truck, contemplating life.

The most common port for the pellets to get into is the one that goes to the purge valve on the engine, because it sucks the fuel vapors from the canister during a purge.

Do those pellets break down over time? Like, if one was sucked up into a hose, does it stay there forever?

No, and yes. In that order.

They just break loose inside the canister and go for a tubal vacation, fulfilling a lifetime pipe dream.
