I am going to be replacing my steering rack soon. Question is, Do the front wheels have to be raised off the ground to replace the rack & pinion if the truck is lifted and I have plenty of clearance to work under the truck? Will it make the job easier to do if the wheels are off the ground or not?

if you can reach everything you will be fine with wheels on the ground
your going to need a alignment afterwards, even if you get it super close. you might get death wobble on the way to the alignment shop if that happens just slow down until it stops wobbling. just accelerate slowly after getting on there, if you take off normal and get the wobble it can catch you by surprise.
