It’s a Havis console. I’m pretty sure this design is discontinued, I think my order was probably one of the last they manufactured. Your best luck is to find one used or old stock somewhere. I’ll try to scramble up a part number for you.

Got the CB antenna mounted finally. Had to make a custom 1/8” steel bracket that supports it from underneath the bumper & behind the tail light. I’m pleased with the final result. Now to run the 18’ coax.
