Is it normal for the transmission to hesitate shifting when under medium to heavy throttle? Or if it's cold?

Not really. Should actually shift fairly firm if everything is in correct working order when under medium or heavy acceleration

Yes, under certain throttle conditions. Under heavy/full throttle it should not hesitate (it is also normal for the later tahoes to not slam into the next gear-to reduce wear and damage). Under moderate throttle it is normal. The transmission is deciding if it can maintain the current gear or requires a down shift. If you are spazzy with the gas pedal it's going to hesitate.

For the most part, the only time I have an issue is when I first start up the truck, and this is an obs btw.

It's cold, it's trying to heat up. I wouldn't be full throttling a cold engine if I was you.

Well obviously, I said for the most part. When cold, it hesitates, and sometimes at full throttle. Just making sure sometimes hesitating at full throttle from time to time isn't an issue.

Sometimes it will shift at a higher rpm than normal. Its not often that it does this. But just every now and then
