Holes in the frame rails

Is this how the vehicle come or is it rust that did this?

The picture is very out of focus, but as bigmike says, those are factory holes. It looks like they're been 'rust-enhanced' a bit, but they're there from the factory.

I will take a better pic today and post it. The 3 small ones on the right look factory but that big one looks weird.
