    Well the mat is removed but not tossed out so I definitely can try that. That's a good idea to trim it back and see if it's enough to prevent a reoccurrence. Neither the floor nor the mat have any positive retention features that I can see.

    Thank you for all the helpful responses!! This place has a really good support network!!! Sadly, after closer inspection I think I'm gonna go back to driver error. I don't think my floor mats are spec, and when I tromped the pedal I believe it wedged into the mat. Here's pics of what it looks...

    My beautiful old '98 Burb was a Manitoba Hydro fleet vehicle in it's previous life and very well maintained.
