I've taken mine off and cleaned it cleared the code and it comes right back on. What might be the cause of this happening? The one I have on it's that old.

Determine if the egr is damaged from carbon scoring on the piston. This will cause it to stick.
Regardless of how clean you got it.
Determine if the controller is functioning correctly.

Replace damaged components.

Means your egr valve may not be keeping up with it commands from the pcm. Most cases a sticking pointless or dirty connector points. A good scanner with bidirectional control can command it to move a certain percentage and also get feed back from it to see if it is moving as commanded. Clean the valve good but don't hey cleaner in the electronics. Push on the pointless to check for free movement and you might actually free it a bit.
