There are a few different ways to do the bumper. I don't know that they make relocation brackets, but people often fab something for the HD Silverado bumper. Another approach people take is to use the 03-06 Sierra bumper, with a little less frame modification, and it provides a different look. I'm sure people who've done it will chime in, and provide more info.

What year bumper do i need for the hd silverado
99-02 hd silverado. All you need to do the conversion is the new top bumper cap, new grille, and hd hood. I'm not 100% sure on the measurement but I believe you cut off your current bumper brackets and move them down 1.5" and re weld them on.
You don't have to buy the hood if your just going for the grille and bumper as your stock 1500 hood will work just fine
