I recently bought a 2013 CPO Escalade with 36k miles; the truck is perfect inside and out, but was a smoker truck. It had been maintained well and looks brand new. My dealer detailed it and used an ozone machine and knocked about 60 per cent of the smell. I took the risk of buying it because of the truck's mechanical and physical condition. It was a LOT of work, but I got rid of the smell completely (but it wasn't so bad that there were stains or accumulations).

I steamed the carpets and then shampooed them with a carpet extractor.
I carefully and lightly steamed the headliner and all other fabric.
I carefully shampooed the headliner using Blue Coral foam, being careful not to soak the fabric.
I cleaned all leather surfaces with Zaino Z-9 and Z-10
I cleaned all the interior window surfaces with glass cleaner
I used a soft brush and/or microfiber towel to clean all other surfaces with ACDelco cleaner, GM 88861401
I treated all the black plastic (dash top, doors, interior quarters) with 303 UV Protectant
I treated all the HVAC outlets (dash, floor) with ACDelco Vehicle Odor Eliminator, 88861431, per TSB # 00-00-89-027I
I treated the exterior HVAC air intake (fans on high, non-recirculate) and then the uptake vents on the driver and passenger sides under dash (recirculating) with Hi-Tech Total Release Odor Eliminator - Fresh Air. I used this sparingly and did several re-treatments because the smell of this stuff hangs around. But it really works. I made sure the rear fans were blowing and that the stuff went through them.
I threw away the floor mats and bought new OEM ones

After all that, there is no residual smoke smell. It's a lot of work and some detailers may not go to these extremes -- you have to clean everything and treat the HVAC. I would have used ozone as the next step, but did not need it.

I got a 2016 Tahoe at the beginning of the year off lease. Someone had smoked in it recently but not at lot and not for a long time. Besides odor, I found a lot of stray ashes all over. Besides the cabin air filter, I got some of the chemical guys leather smell spray (about $10) on Amazon. Its crazy strong. A couple sprays in my garbage bins and they smell like they are made out of fresh leather. I put a little under the seats and after the leather smell faded in a week, no more cigarette smell.

I don't think my truck has a cabin filter.

I was at a recent seminar That was put on by Turtle Wax, They have a new product called POWER OUT! ODER-X. They have a line of POWER OUT! ODER-X products. Carpet Cleaner, Leather Cleaner and Deodorizer.
Look for the WHOLE CAR BLAST kit..

A used car dealer taught me a trick I've used. Put a pan of ammonia on the floor of the vehicle overnight. The next day remove it from the vehicle and air it out good. Pulls the odor out of the headliner. Sometimes a second dose is needed. Cheap and it works.

Ozium or Chemical guys makes some good stuff.
